As part of our pre-screen process, our administrative team will create an account for you and send an intake form. This form is where you will provide important information regarding medical history, development, and your primary areas of concern. During this step, we will also work with you to schedule your evaluation and treatment times. We then carefully review the completed form to ensure everything is in place. Please note that this form must be submitted prior to your first session. Once we receive it, we are ready to begin therapy!
